Social Determinants of Health in Primary Care Settings

In this presentation, Mid-America MHTTC provides a rationale to address Social Determinants of Health in primary care settings. The presentation provides a broad overview of the topic and serves as introduction to future presentations of specific conditions by which people live, work and age. In particular, participants of this presentation will:

  • Define social determinants of health, health equity and health disparities.
  • Describe the impact of social determinants on health outcomes.
  • Understand the importance of assessing for common social determinants of health in primary care settings.
  • Identify actionable steps to screen and refer to community supports for social determinants of health.

 

 

Orientation to Z Codes

This presentation provides an orientation to Z-codes and how they are utilized in primary care settings to improve care. You will:​

  • Learn what Z-codes are​.
  • Learn how Z-codes are predictive of health status.​
  • Learn to utilize Z-codes to track, report, and make referrals to address social determinants of health.

 

 

Food Insecurity

This presentation will introduce food insecurity. You will learn:

This presentation provides an orientation to employment as a social determinant of health. You will:

This presentation provides an orientation to housing instability as a social determinant of health. You will:​

  • Learn what housing instability includes (e.g., cost burdened, overcrowding, forced/multiple moves, homelessness).
  • Learn how housing instability affects physical and mental health.
  • Learn how to assess for housing instability and make referrals for appropriate supports.

Addiction: Gambling, Alcohol, Drug Abuse

This presentation provides an orientation to addiction as a social determinant of health. Attendees will:​

  • Understand gambling as an addiction.
  • Define how gambling can interfere with a persons physical and mental health​.
  • Review screenings and assessments for gambling​.
  • Identify referral sources for gambling issues.

 

Internet Access

This presentation provides an orientation to internet access as a social determinant of health. Attendees will:​

  • Learn how internet access is related to physical and mental health.
  • Learn how to assess for internet access and problems related to health literacy.
  • Learn how to adjust communication to address low levels of health literacy.

 

 

Transportation and Social Support

This presentation provides an orientation to transportation and social support as social determinants of health. Attendees will:​

  • Discuss the effects of lack of reliable transportation and social support on health and healthcare utilization.
  • Identify two screening tools to assess for access to transportation and social support.
  • Identify local and national resources to address lack of transportation.

 

 

Domestic Violence

This presentation provides an orientation to domestic violence as a social determinant of health. Attendees will:​

  • Learn the effect of domestic violence has on health and health care utilization.
  • Learn how to assess for domestic violence and how to address problems related to domestic violence.
  • Learn how to make referrals for appropriate services for additional support.

 

Suicide and Self-Harm

This presentation provides an orientation to suicide and self-harm as social determinants of health. Attendees will:​

  • Discuss the effects of self-harm, suicidal thoughts and behavior on health.
  • Review screenings and assessments for self-harm and suicidal thoughts.
  • Discuss safety planning and resources related to promoting safety.

This presentation provides an orientation to pediatric sleep as a social determinant of health. Attendees will:​

  • Understand the importance of pediatric sleep for health and well-being​.
  • Describe the impact of social determinants on pediatric sleep.
  • Understand the importance of assessing for common sleep concerns within a social determinants of health framework in primary care settings.​
  • Identify actionable steps to screen and refer to community supports for social determinants of health.

 

Adolescent and Adult Sleep

This presentation provides an orientation to adolescent sleep and adult sleep as social determinants of health. Attendees will:​

  • Learn how sleep is related to physical and mental health. ​
  • Learn how to assess for sleep problems​.

Adherence as a Social Determinant of Health 

This presentation provides an overview of the role of adherence as a social determinant of health. Attendees will:​
