LEARNING OBJECTIVES

  • Practice Improvement - Implement new person-centered planning practices that will foster positive outcomes for those served.
    • Integrate person-centered values into practice,
    • Cultivate connections inside the system and out for people
    • Ensure quality improvement and monitoring are consistent with person-centered principles
  • Compliance - Implement the framework to ensure compliance with rules, while still allowing for creativity and innovation.
  • HCBS Rule Implementation
  • Conflict Free Access and Planning Implementation
  • Documentation (quality person-centered goals and objectives)
  • Quality measures
  • Policy Revision and Improvement - Implement policies and procedures that are transparent, consistent, and person-focused.
  • Engagement of participants and family members to improve processes (focus groups, surveys)
  • Engagement of case managers/support brokers to improve processes
  • A process that is accessible to individuals and families from a range of incomes, education, and race/ethnicity

 

CERTIFICATES

PRESENTER

Tracy Vincent

Tracy Vincent has worked in the disability field for 20 years. Currently, she’s the Resource Analyst for the Michigan Developmental Disabilities Council. She responsible for the design and development, implementation, and evaluation of the states Peer Mentor Program, a Medicaid service for individuals with developmental disabilities. Tracy is an experienced planner, trainer, negotiator, data manager, and curriculum developer. Ms. Vincent is the Lead Facilitator of Michigan’s Community of Practice for Supporting Families. As the CoP facilitator, Tracy provides leadership and oversight of the CoP’s initiatives. She’s been a Charting the LifeCourse Ambassador for five years.

 

 

The Great Lakes MHTTC is offering this training for individuals working in HHS Region 5: IL, IN, MI, MN, OH, WI. This training is being provided in response to a need identified by Region 5 stakeholders.
