In this presentation, Mid-America MHTTC provides a rationale to address Social Determinants of Health in primary care settings. The presentation provides a broad overview of the topic and serves as introduction to future presentations of specific conditions by which people live, work and age. In particular, participants of this presentation will:

• Define social determinants of health, health equity and health disparities
• Describe the impact of social determinants on health outcomes
• Understand the importance of assessing for common social determinants of health in primary care settings
• Identify actionable steps to screen and refer to community supports for social determinants of health
